Catholic Charities is the social service arm of the Catholic Diocese of Winona-Rochester serving the twenty southernmost counties of Minnesota. For over 75 years, Catholic Charities of Southern Minnesota has served the poor and marginalized regardless of age, gender, ethnic background, or faith tradition.

Catholic Charities of Southern Minnesota is seeking a full-time Adult Advocacy Case Manager in our Winona office. We are seeking a dedicated and compassionate Case Worker to join our team. This role requires a strong commitment to helping clients navigate challenges and access necessary resources for their well-being.

The Case Manager advocates for and acts on behalf of clients deemed incapacitated by the court. This position is responsible for case planning, obtaining and implementation of services, timely maintenance of case notes and files, and adherence to court deadlines. The Case Manager will have regular contact with clients and providers and share the responsibility of the on-call service.

Job Description:

Catholic Charities is looking for a case manager to provide case management services as defined by Minnesota Statute or service agreement to the clients of the Adult Advocacy Program with a full caseload,. T

The ideal candidate should be committed to working with individuals in a manner that promotes their dignity and sense of self-worth, be creative, organized, show ability to multi-task and possess a real passion for helping others. Adhere to deadlines and possess strong verbal and written communication skills.

Responsibilities:

  • Advocate for and act on behalf of person’s as arranged by service agreement or deemed incapacitated or incompetent
  • Maintain a caseload based on clients’ needs
  • Complete reporting responsibilities for timely assessments, case planning supervision and implementation of service, reporting, case notes and service deadlines
  • Communication with other service providers
  • Completes responsibilities to clients, families, and District Courts including submitting Personal Well Being reports, attending and testifying at hearings.
  • Responsible for filing, correspondence with other service providers, documentation via case notes, and opening and maintaining files for new clients.
  • Conduct client visits by personal needs shopping and/or with clients
  • Assist with obtaining housing, as needed by working closely with other service providers
  • Intervenes in situations as needed so the client’s best interests are serviced
  • Provides backup support to the program coordinator as needed
  • Collaborate outside agencies and providers to ensure adequate and appropriate services
  • Shares and maintains on-call services
  • Other duties as needed

Experience

  • Strong interpersonal skills with the ability to communicate in a direct straightforward manner both verbally and written
  • Excellent organizational skills and attention to detail in documentation and reporting.
  • Ability to work collaboratively within a multidisciplinary team environment.
  • Must possess clear, unbiased thinking and decision making skills in stressful situations.
  • Detail oriented, able to prioritize work, and resolve problems efficiently
  • Demonstrate maturity, flexibility, creativity, initiative, organizational skills, and work effectively with others
  • Be efficient, able to problem solve, and demonstrate leadership and collaboration.
  • Ability to interact successfully with people with a diverse background, persons with diagnosed mental illness, disabilities, or substance abuse/chemical dependency

Required

  • A minimum of a four-year degree in a human service field or equivalent education and experience.
  • Ability to interact successfully with people with diverse backgrounds and with persons diagnosed with mental illnesses, physical disabilities, intellectual disabilities or substance abuse/chemical dependence.
  • Math and computer proficiency in spreadsheet, databases and word processing.
  • Sound teamwork skills.
  • Great attention to detail and accuracy.
  • Must demonstrate maturity, flexibility, creativity, initiative, organizational skills, the ability to work effectively with others, and the ability to think systematically.
  • Ability to communicate information, both in writing and verbally, in a direct straightforward timely manner.
  • Must be detail oriented, able to multi-task, prioritize work, and to resolve problems.
  • Must possess clear, unbiased thinking and decision making skills.
  • Commitment to furthering the mission of Catholic Charities of Southern Minnesota.
  • Minimum of 2 years of experience working with persons with mental illness, developmental disabilities or other vulnerable adults.
  • Must own automobile with valid driver’s license and current insurance
  • Must be able to pass extensive background check
  • Occasional travel is required

Pay: From $21.15 per hour
